This Cardholder Agreement (this "Agreement") sets forth the terms of your Prepaid Card. Please read it carefully and keep it for your records. If you do not agree to these terms, do not use the card; o If you want to cancel, call Customer Service at 52-812-046-6231 or 1-855-879-8344. Otherwise, your acceptance and / or use of the Card will be evidence of your acceptance of these terms.

NOTE: THIS SETTLEMENT REQUIRES THAT CERTAIN DISPUTES BE RESOLVED THROUGH BINDING ARBITRATION, RATHER THAN THROUGH A JURY TRIAL. THE TERMS OF THE ARBITRATION CLAUSE CAN BE FOUND AT THE END OF THIS AGREEMENT.

Definitions. In this Agreement, the words “you” and “your” refer to the Cardholder. "Bank", "we", "us" and "our" refer to Axiom Bank, N.A. ,, the issuer of the Card, or anyone to whom we assign our rights. “Card” refers to the Franchise Card issued to you. Consent. Individuals who believe they have received this Card without their consent will be entitled to a full refund of the costs charged to the Card. Individuals can claim their full balance by visiting dailypay.me or by calling the number on the back of the Card. ID. To assist the government in its fight against terrorist financing and money laundering activities, we are required by federal law to obtain, verify, and record information that identifies each person who obtains a Card. When you apply for or agree to receive a Card, you authorize the issuer of the Card to provide us with your name, address, date of birth, and other information that will allow us to identify you. We may also request proof of your driver's license or other identification documents, and we may use resources such as credit bureau reports or other means to verify your identity.

Use of the Card. This card was issued in your name as the only current option for you to receive the funds that have been loaded onto it, so you will need to keep it. However, if you do not want to use this card, you can request a check or an alternative disbursement method from those listed in the rate table, at no cost. The card cannot be used until it has been validated. You can validate this card by calling the toll-free number listed on the back, registering the card on the website shown on the back, or making transactions with the card. Charge rates for the activity will not be applied to the card until it is validated. However, an inactivity fee will be charged to the card according to the rate table after the applicable period of inactivity, even if the card has not been validated. If the card is validated after an inactivity fee has been assessed, and before the funds are returned to the appropriate regulatory authority, you may request that the inactivity fees be reimbursed to the card by calling the toll-free number listed at back of the card. You may use your Card to purchase goods and services anywhere MasterCard® debit cards are accepted and to obtain cash at financial institution ATMs that display the MasterCard®, Pulse® or Maestro® name and / or logo. Each time the Card is used to purchase goods or services or to withdraw cash at ATMs, you authorize us to charge that amount (and any applicable fees) on the available balance on your Card. You may not assign or transfer your Card to another person for their use. You will be required to enter your personal identification number (“PIN”) to access cash at ATMs and to purchase goods or services at some sales terminals (“POS”). Check the activation label on your card for your temporary PIN. You will need to change your temporary PIN immediately by calling the Customer Service number at 52-812-046-6231 or 1-855-879-8344. You agree not to disclose your PIN to others.

ATM receipts. You will be able to obtain a receipt the moment you make a withdrawal with your Card by using one of our ATMs.

Balance and Transaction Information. Information about the current available balance on your Card and a description of recent transactions can be obtained by calling Customer Service at 52-812-046-6231 or 1-855-879-8344 or visiting the website on the back of your card.

Limitations. Subject to your available balance, you can use your Card to make ATM withdrawals and purchase products or pay for services up to a total amount of $ 3,500 per day. You will not be able to make more than five ATM transactions or twenty purchase transactions in a single day. For security reasons, we may further limit these amounts.
